Application for a U.S. Passport
[pdf]

On June 1, 2009, the U.S. government implemented the full requirements of the land and sea phase of WHTI. The proposed rules require most U.S. citizens entering the United States at sea or land ports of entry to have a passport, passport card, or WHTI-compliant document.

CHILDREN: U.S. citizen children under the age of 16 will be able to present the original or copy of their birth certificate, or other proof of citizenship such as a naturalization certificate or citizenship card.

Groups of U.S. citizen children ages 16 through 18, when traveling with a school or religious group, social organization,

or sports team, will be able to enter under adult supervision with originals or copies of their birth certificates or other proof of citizenship. See the Department of Homeland Security's GetYouHome.gov for more information on the changing travel requirements

***FREQUENTLY ASKED QUESTIONS***

NOTEYou will need to submit a photocopy of the identification document with each application--(i.e., driver's license; government ID, military, valid passport/card; certificate of citizenship/naturalization, etc.). The photocopy must be the same ID document that you will present when you apply.

For minors under the age of 16:  When applying for a minor, you will need to submit a photocopy of both parents ID document for each minor's application.

The photocopy MUST be on plain white, 8-1/2 x 11" standard paper stock, showing the front and the back of your ID.  Photocopy MUST contain images on only one-side of each page submitted.

The paper should be free of other images and/or markings.

The 8-1/2 x 11" paper size cannot be substituted with a larger or smaller size paper. You may enlarge the image of your ID on the 8-1/2 x 11" page, but you may not decrease the size of the image.
